We are happy to hear from you. Please contact us using the information below:

Please arrive 10-15 minutes early for your appointment for registration and to fill in a health questionnaire.

When you come for your appointment, please bring the following:

• Medicare card & Referral letter
• Have your insurance information
• If possible, copies of any operation records, medical records, x-rays, MRIs, CT scans and so on from prior doctor visits (optional)
• If you have had Weight Loss Surgery elsewhere, please bring a copy of your operation report

If you need to cancel an appointment

please call the office during business hours and provide at least one day’s notice so that we may offer your appointment time to another patient on our waiting list.

We recognize that your time is valuable and make every effort to stay on schedule. Occasionally, emergencies or patients requiring additional care may cause delays beyond our control. We appreciate your understanding and apologize for any inconvenience if you experience a wait.
